Payment request form for guests + facilitators

This form allows faculty and instructors to request honorarium payments for reviewers, guest lecturers and workshop facilitators. This form can also generate thank you notes to send to guests. The output of a submission will result in a thank you note with a payment link enclosed sent to your guest, or a request that a staff member reaches out to your guest to provide a payment link. This payment link outlines options for guests to either receive payment directly, or to direct the value of the honorarium being offered to support students and/or student-led initiatives.
