Please indicate whether or not you have remained opted in to the health insurance plan offered by CUSA (undergraduate students) or the GSA (graduate students). The CUSA and GSA health insurance plans offer comprehensive travel coverage for students.

Insurance Policy Declarations Page(Required)

If you have opted out of the health insurance plan offered by CUSA (undergraduate students) or the GSA (graduate students), you are required to provide a copy of the Declarations Page (also known as a Summary of Benefits) of the insurance policy that you are utilizing for your travel activities. The Declarations Page (or Summary of Benefits) should summarize the coverage provided in your policy. The file you upload will be provided to the Office of Risk Management for review.

Travel Registry Entry(Required)

Students, faculty/instructors and staff are required to prepare a Travel Registry Entry for any travel they participate in that pertains to courses, studios, research and fieldwork. Please refer to the instructions file provided in order to prepare your entry and upload your screenshot.

International SOS(Required)

All individuals travelling are encouraged to utilize the travel support provider resources made available to the University through International SOS, a dedicated website is available to register students’ travel. Students can sign up for trip related notifications and receive tailored support while abroad. More information on International SOS can be accessed here: https://myportal.internationalsos.com/WebPortal/MyOrganisation.
